You can also obtain a hard copy of the application at the Vital Statistics window in the Municipal Building (please note the Vital Statistics hours of operation).
Use this form to apply for:
- Death Certificate(s)
Death certificates are filed in the township where demise was officially pronounced.
To obtain a certificate, please remit your application, payment, and supporting documents in person at the Vital Statistics window in the Municipal Building.
The immediate family or Executor/Executrix must show identification in order to secure a certified raised sealed copy.
(Immediate family members includes the following ONLY: Mother, Father, Sister, Brother, and Children).
Fees & Payment
There is a $25 fee for the initial certificate and a fee of $5.00 for every copy requested thereafter.
Personal checks, money orders, and cash are accepted.
The required documents to obtain a Death Certificate(s) are:
- A valid driver's license or state issued identification and/or a valid passport.
- The fee is $25.00 and $5.00 for each additional Death Certificate.
