The Chronic Disease Self Management Program (CDSMP) is sponsored by the NJ Department of Health and Senior Services. This program was founded as part of their strategic plan to assist minorities -- who have been proven to disproportionately affected by chronic diseases.

The Willingboro Senior Services department recognized that there needed to be more community outreach in order to identify the barriers to minorities seeking disease treatment, and to help provide education on healthcare decision making.

To accomplish this goal, Willingboro Senior Center's director, Reva Foster, Johnathan Walker, and Helga Luman pursued training and became Master Trainers of the program on February 1st, 2008. The Willingboro Senior Center is now licensed to conduct the CDSMP program at our facility.

The aforementioned leaders conduct trainer-to-trainer (Peer Leaders) workshops in collaboration with the New Jersey Black Issues state organization. As a result, the CDSMP is able to multiply.
