You can also obtain a hard copy of the application at the Vital Statistics window in the Municipal Building between the hours of 9:00 am and 5:00 pm. For more information, contact the Registrar at 609-877-2200, ext. 1030.

Instructions

Death certificates are filed in the township where demise was officially pronounced.

To obtain a certificate, please remit your application, payment, and supporting documents in person at the Vital Statistics window in the Municipal Building.

The immediate family or Executor/Executrix must show identification in order to secure a certified raised sealed copy.

(Immediate family members includes the following ONLY: Mother, Father, Sister, Brother, and Children).

Fees & Payment

There is a $25 fee for the initial certificate and a fee of $10.00 for every copy requested thereafter.

Personal checks, money orders, and cash are accepted.

Documentation

The required documents to obtain a Death Certificate(s) are:

  • A valid driver's license or state issued identification and/or a valid passport.
  • The fee is $25.00 and $10.00 for each additional Death Certificate.

Free viewers are required for some of the attached documents.
